By default, the value of a checkbox will be sent via a form only when it's checked.

This jQuery plugin fixes this behavior by enabling an unchecked value to be added which will be sent whenever the form is submitted, if the checkbox has been left unchecked.

A demo page is included with the plugin, to serve as a starting point for your Web forms.
